• An Online Payment Fee will be charged if this invoice is paid online, with the optional Zelle payment method. No fee will apply.
• Registration required. This plan covers all your drain stoppage needs. 2 drains maximum per service request. Kitchen sink, laundry, bathroom sink, tub, shower, toilet, above-ground mainline drains or roof vent stoppages, etc. A $150 service charge will apply. An optional 30-day, 60-day, 90-day, warranty is available upon request

• HOLD HARMLESS: In requesting any drain cleaning, sewer cleaning, or rooter services, it is indicated. I understand that the condition of the drains and plumbing fixtures may have deteriorated by age, exposure to acid or other chemicals, or normal wear and tear. Therefore, in consideration of the services requested, I hereby release GEE'S DRAIN ROOTER SERVICE LLC, from any claims of property damage, including consequential damages, in connection with or arising out of the services requested, I further agree to indemnify and hold GEE'S DRAIN ROOTER SERVICE LLC, harmless against any such claims made by any third party.
